Request for Enrollment Verification Letter form Requests the Registrar's Office to provide a verification letter for special cases: e.g. insurance, loan deferments, current classification, anticipated graduation date for clinical students, etc.
Transient Request form (Complete this form to request permission to take a class or classes at another school) Franciscan Missionaries of Our Lady University students may pursue classes at another accredited institution with the approval of the Dean of the appropriate school and the Registrar. Failure to obtain this approval may result in the denial of credit. Students must be in good academic and financial standing with Franciscan Missionaries of Our Lady University. Students seeking or receiving any form of financial assistance must speak with a Financial Aid Officer regarding the effects of enrolling at multiple institutions on their aid eligibility. At the conclusion of the semester, the student has the responsibility to have an official transcript mailed to the Registrar’s Office. If the transcript is not received, the student will not be eligible to register for future classes or obtain a copy of his or her transcript.
Non-Directory Information Release form (Student who wants to allow a parent or other to access their records) In accordance with the Family Educational Rights and Privacy Act (FERPA), this form allows students to grant parents, guardians, spouses, and/or others access to their educational records maintained by the Registrar’s, Business and Financial Aid Offices and other student records maintained by other entities on the Franciscan Missionaries of Our Lady University campus.
