Student Conduct Report Form

This form may be used for Student Code of Conduct, Academic Honesty, or general informational reports. Information on these different types of reports are included below.

Student Code of Conduct Violation (non-academic violations)

Non-academic violations are behaviors that include, but are not limited to, alcohol, disorderly conduct, drugs, hazing, harassment, physical violence, or other violations of university policies, regulations, or rules. These reports are received and managed by The Dean of Students and should be submitted due to alleged policy violations as described in the Student Code of Conduct within the Student Handbook.

Academic Honesty Violation

Academic violations are behaviors that include, but are not limited to, cheating, plagiarism or falsifying academic related information. General classroom misbehavior is considered a non-academic violation. These reports will be sent to the appropriate academic School for follow-up and should be submitted due to alleged policy violations as described in the Academic Honesty policy within the Student Handbook.

Informational Reports
Report behaviors that you perceive as concerning, worrisome, or threatening. Examples include bizarre/disjointed thoughts or behaviors, academic stress, disruptive class behavior, suicidal thoughts, etc. These reports are received by Student Affairs and may be routed to departments as necessary. Please describe the situation in as much detail as possible.

Incident Description

Please provide a detailed description of the incident/concern using specific concise, objective language. If you have listed an individual as an involved person, he or she should appear at least once in the narrative. For matters involving Conduct violations, please provide as much detail as possible in describing the behaviors and activities that occurred. The use of direct quotes, including profanity or abusive language, is encouraged. Avoid using pronouns when multiple individuals are involved.

What to Do

  • Provide a detailed description of the incident/concern using specific, concise, and objective language. (Quotes are always helpful!)
  • Answer the Who, What, Where, When, Why, and How.
  • Include dates, times, details and facts.
  • include the name(s) of personnel you contacted regarding this incident and all actions taken.
  • Attach any necessary images or supplemental documents.
